HEADTEACHERS, PSHE LEADS, YEAR HEADS
Warwickshire School Health and Wellbeing Service - updates
Prior to, and during the summer holidays, the service has been actively promoting the School Entry online health needs questionnaire. This health needs questionnaire enables parents to inform us of any health concerns they have for their children.
Early in September we will send out information and consent paperwork to all schools for parents/carers regarding the online Health Needs questionnaire which is offered to year 6 and year 9 students. For both year 6 and year 9, consent is considered to be given unless a parent/carer specifically decides to opt out of the process. The information gathered through the completion of the online health questionnaire enables us to understand the health needs of students. Following analysis of the data we will contact schools to arrange a meeting with the head teacher/PSHE lead/year head. We will then jointly develop a plan for how we can work together to address the identified health needs.
We would like to assure you that all information is held securely and only anonymised information is shared with partner agencies.
